Designed for the frail elderly and disabled who live at home, this program provides enjoyable, supervised activities, medical care, a superb hot meal, and door-to-door transportation— all in the upbeat, ultra-modern setting of Parker's Community Health Center in Lake Success.
Parker's Adult Day Health Care program is the single largest medical model program in Queens and Nassau, and provides two sessions, 10 AM to 3 PM and 3 PM to 8 PM, Monday through Friday, and Sunday.
There is a separate program for those with memory loss*, and a special Chinese cultural program.
On-site medical care is available from physicians, nurses, dentists and therapists. There are also specialists in foot care, hearing, vision and nutrition. Parker's medical care may also be available to family members of participants.
Medicaid and private pay are accepted. In some cases, long-term care insurance, Workmen's Compensation, or the VA may cover day care. Financial Entitlement Coordinators are available to answer your questions about insurance eligibility.
