Home Health Care

The Kessel Long Term Home Health Care program brings the unmatched experience and expertise of Parker Jewish Institute for Health Care and Rehabilitation directly to your home. Parker's "nursing home without walls" is available to residents of Queens, Brooklyn and Nassau Counties. You may apply to this program after discharge from a hospital or long term care facility, or directly from home.  


Home care services include:

• comprehensive assessment by a registered nurse
• individualized nursing care plans
• physical and occupational therapy
• speech therapy and audiology testing
• nutritional planning
• social work counseling
• homemakers and housekeepers
• home health aides
• personal care workers
• medical supplies and equipment
• 24-hour telephone availability
• personal emergency response systems (PERS)

Additional assistance may be available for:

• housing modifications
• home maintenance
• moving services
• social day care
• respite care
• congregate meals

A number of services may be coordinated through this program, including: medical transportation, laboratory work, x-rays, drugs, dentistry, ophthalmology, podiatry, medical day care, and discharge planning.

Medicare, Medicaid, private pay and many insurance plans are accepted.
