If you or someone close to you is very ill, and wants to remain home with loved ones but needs help with living as well, and as long, as possible and are not wanting to “give up” or give up treatment, there’s a special kind of care you should know about.

Care Choices
Through Care Choices, the Centers for Medicare & Medicaid Services (CMS) will provide a new option for Medicare beneficiaries who qualify for coverage under the Medicare or Medicaid Hospice Benefit, the option to elect to receive supportive care services typically provided by hospice and continue to receive curative services at the same time. Under the new model patients with certain end-stage-illnesses are not required to forgo curative care in order to receive select hospice services. This new model is part of a larger effort to deliver better care, and put patients in the center of their care.

Who Can Enroll?
Individuals who wish to receive services under this new model must fall into certain categories:

  • Must be diagnosed with certain terminal illnesses (e.g., advanced cancers, chronic obstructive pulmonary disease, congestive heart failure and human immunodeficiency virus/acquired immune deficiency syndrome);
  • Must meet hospice eligibility requirements under the Medicare or Medicaid Hospice Benefit;
  • Must not have elected the Medicare or Medicaid Hospice Benefit within the last 30 days prior to their participation in the Medicare Care Choices Model;
  • Must receive services from a hospice that is participating in the model; and
  • Patient lives in a traditional home now and continuously for the last 30 days.

The Care Choices Team works with your physician(s). There is no need to change your doctors. Your primary provider remains in charge of your care.

How Can This Help Me?
Our Care Choices Team provides patients with end -stage –illness and their families with a cocoon of support services can help them tolerate treatment for better outcomes, specifically:

  • Education/assistance on symptom management
  • Communication
  • Identifying/clarifying goals, support
  • Care coordination, case management
  • Improving the quality of life for patients

Patients and their doctors continue to direct the care of the patient in the Care Choices model.

What Services are Covered?
The care services provided through the model are available 24 hours a day, 7 days a week 365 days a year. These services are the following:

  • Care Coordination, Case Management, and Shared decision making
  • Nursing services
  • Pain and symptom management
  • Social Work Services
  • Hospice Aide Service
  • Hospice Homemaker Services
  • Volunteer Services (direct services only)
  • Chaplain Services
  • Bereavement Services
  • Nutritional Support Services
  • Respite Care

Patients remain subject to any relevant Medicare cost-sharing requirements. incurred as a result of curative care treatments.
