Parker Institutes Kessel Long Term Home Health Care Program helps older adults receive medical care at home while remaining independent. This program brings individualized medical and nursing services directly to people who are homebound because of physical limitations. Professional staff members visit patients regularly to assess, develop treatment plans and provide ongoing care.
Patients can take advantage of a variety of services and support, such as:
- nursing care
- physical and occupational therapy
- speech therapy and audiology testing
- respiratory therapy
- nutritional planning
- social work counseling
- medical supplies and equipment
The Program offers additional assistance for:
- housing modifications
- home maintenance
- moving services
- social day care
- respite care
- emergency alarm response services
- congregate meals
Queens and Nassau County residents may apply to this program after discharge from a hospital or long term care facility. Also, an application can be made directly from home. Individuals must have a demonstrated need for skilled nursing or health-related care.
