Comprehensive care plans are developed to meet each patient’s individual needs. The Hospice interdisciplinary team includes physicians, nurses, social workers, clergy members, bereavement counselors, dietitians, therapists, and volunteers.

Every patient has access to a range of services and support including:

• skilled nursing
• physician services
• nutritional services
• therapy services
• spiritual counseling
• inpatient respite
• short-term inpatient care
• medical equipment and medications
• home health aides
• 24-hour a day on-call services

Bereavement services are available from the time of admission into hospice through the bereavement period to help with grief and other issues of loss.

Hospice care is reimbursable under Medicare, Medicaid and most private insurance.
