Indian Culture Unit

Our Indian Culture Unit is the first of its kind serving East and West Indian Older adults living in Nassau and Queens. This dedicated Indian Cultural Unit provides specialized care for Indian patients. Staffed by Indian physicians and nursing staff fluent in Indian languages; Bengali, Gujarati, Hindi and Punjabi, we deliver a high standard of care that is culturally relevant. There is Indian-inspired artwork and décor, activities and entertainment featuring TV, movies, newspapers, music, and religious services, so patients remain connected to their traditional lifestyle. Traditional Indian vegetarian cuisine is served daily.

Hospice/Palliative Care

Pallitive and hospice care both focus on comfort and quality of life. Pallitive care can be proivided at any stage of serious illness alongside treatment. Hospice care is for individuals with a prognosis of six months or less and focuses solely on comfort.

Palliative Care
Our team of medical professionals can provide personalized care for you if you suffer from pain, stress or other symptoms due to a serious illness. Serious illnesses may include cancer, heart disease, lung disease, kidney disease, Alzheimer’s, HIV/AIDS, amyotrophic lateral sclerosis (ALS), multiple sclerosis, Parkinson’s and more.

Parker At Your Door

Parker At Your Door is a physician-led house call programdelivering personalized primary care directly to the homes of older adults who arehomebound. In addition to medical house calls, the program offers specialized services.
Medical House Call Program
Connecting patients to services they need:
Post-discharge and transitional care: following hospitalization or rehabilitation
Preventive and routine care: including annual wellness visits, physical exams and immunizations
Remote care options: such as telehealth and remote patient monitoring
Care coordination: including referrals and collaboration with specialists
Diagnostic services: including laboratory tests, EKGs and pulse oximetry
Supportive services: such as ordering medical equipment, home health care as well as palliative care
Free Dementia Care Services Through the Dementia GUIDE Model
Guiding an Improved Dementia Experience (GUIDE) offers a personalized, coordinatedapproach to dementia care—at no cost to families. Each person is paired with adedicated Dementia Care Navigator who connects them to essential services.
Medicare, Medicaid, managed care, private pay and many insurance plans are accepted. Serving communities across the Bronx, Brooklyn, Queens, Manhattan, Nassau, Suffolk and Westchester.

Willing Hearts Helpful Hands

Willing Hearts Helpful Hands is a uniquerespite and resource program for caregiversof loved ones with Alzheimer’s diseaseor other dementias. Providing guidance,support and a caring hand so caregiverscan continue their journey with strength.
Services Include:
Caregiver support groups and workshops
Offering support groups on caregiving forloved ones with Alzheimer’s disease orother forms of dementia. These events are held virtually or in-person.
In-home companion care / Respite program
If you’re a family caregiver for someone with Alzheimer’s or other dementias, you may need occasional support. Our respite scholarship program offers short-term care during emergencies, illness or caregiver breaks—including vacations. Available to caregivers in Nassau and Suffolk, this program ensures your loved one is cared for when you can’t be there, giving you peace of mind and time to recharge.
Assistance with resources and planning
Whether you are a friend or a family member, caringfor an older adult can be a challenge. Willing HeartsHelpful Hands volunteers offer scheduled care andsupervision for your loved one, giving you time to focuson your own needs and well-being.
Educational events and memory cafes
These events and cafes offer a welcoming spacefor people with memory loss and their care partnersto enjoy snacks, live entertainment and meaningfulsocial time. There’s no agenda—just a chance to relax,connect and have fun with others who understand.
